We accept referrals from pediatricians, school counselors, therapists, teachers, and hospital discharge teams. For therapy, psychological evaluations, and specialty programs. Children, teens, adults, couples, and families. We respond within 1 business day.

What you can refer for.

CCFC is an outpatient practice with specialty-level programs. We see a wide range of presentations and can typically match your patient to the right level and type of care.

Therapy

Individual therapy for children (ages 3+), teens, adults, couples, and families. Specialties include anxiety, OCD, trauma (TF-CBT, EMDR, CPT, Brainspotting), depression, ADHD, autism-related social and behavioral challenges, grief, life transitions, LGBTQ+ concerns, and more.

Psychological Evaluations

Comprehensive evaluations for ADHD, autism (ADOS-2), learning disabilities, gifted and twice-exceptional, kindergarten readiness, accommodations (504, IEP, college), and adult diagnoses. Pre-adoption assessments also available. Report turnaround: 4 to 6 weeks.

Specialty Programs

Structured programs with pre and post measures, specific goals, and matched protocols. DBT skills classes and Teen Thrive (comprehensive adherent DBT), Anxiety and OCD Program (ERP, SPACE, CBT, ACT), Disruptive Behaviors Program (PCIT, PMT, CPS), and group-based parent classes.

Who is a good fit?

We can support most outpatient presentations. Here are the situations referring providers most commonly send to us.

Common pediatric referral

Child with ADHD, anxiety, or behavioral concerns at school

Eval to confirm diagnosis plus therapy to address what the eval finds. Or therapy alone if diagnosis is established.

Common school counselor referral

Teen with school refusal, self-harm history, or emotional dysregulation

May be appropriate for our Anxiety and OCD Program or Teen Thrive depending on severity and presentation.

Common therapist referral

Client needing higher-structure outpatient or specialty program

We can absorb clients stepping up from weekly therapy or stepping down from IOP, PHP, or residential. We coordinate with previous providers.

Common ER / hospital referral

Discharge planning for a child or teen who needs outpatient follow-up

We prioritize post-discharge referrals and will contact the patient within 1 business day. For crisis or higher LOC, see our higher-level-of-care partners below.

Common teacher / administrator referral

Student whose behavior or emotional state is affecting learning

We often start with an evaluation to understand what is driving the behavior before beginning therapy. School coordination available when the family consents.

When we are not the right fit

Active suicidality, psychosis, or acute stabilization needs

We are an outpatient practice. For patients requiring crisis stabilization or inpatient care, please refer to the appropriate level of care. We are happy to receive a referral after stabilization.

What happens after you refer.

We receive your referral

Referrals arrive by this form, secure fax, or secure email and are reviewed by our intake team, typically the same business day.

We contact the patient

Our intake team reaches out to the patient or family directly within 1 business day to schedule.

We coordinate when authorized

With a signed release from the patient, we can communicate care updates back to your office.
